Does an amended tax form that you sign in court have to be copied and put into your court file?

Say I want to let my ex claim the children on past tax forms. I sign a tax form for him in court. Isn't a copy of that tax form suppose to go into our court records and shouldn't the court agreement have on it everything that was written on the tax paper as well?
